By signing below, you are consenting to yourself/ and the group of children you facilitate to utilizing online platforms for virtual programming.
In order to participate in the virtual programming, you/the children you facilitate will need to access the following:
• A computer, mobile phone, or tablet device with access to the Internet, HIGH SPEED internet is preferred to avoid glitches and interruptions. Victory is not responsible for participants that cannot access videos due to slow internet access
We will use the custom online platform for the following program activities:
• Implementation of virtual programming
Our commitment to keeping the people we serve safe is always our number one priority. Victory Kid Sports will actively monitor participant information by, among other things, maintaining control of, and access to, the data collected and prohibiting re-disclosure of participant information.
Further, all activities online must comply with Victory Kid Sports’s safety policies, including, but not limited to, the Code of Ethics, Code of Conduct, Program Quality Assurance and Zero Tolerance policies.
Victory Kid Sports shall not be responsible for any injury, accident, loss, misfortune or damage of any kind (to property or person) arising from the participation in virtual programming, use of virtual programming platforms and/or relating to any aspect of the above services.
By signing and submitting this consent form, I agree to the information listed above. In addition, I agree to be contacted by a Victory Kid Sports representative for an exit survey after the program is over.
